1.) Are enrollment
cards required?
No, enrollment cards are unnecessary. However, drivers are
expected to complete a
Driver Information Sheet.
This form is kept on file at the motor carrier and is only
submitted to the insurance company in the event of a claim.
2.) What does each covered driver receive?
Each independent contractor receives:

Wallet sized identification card

Bearer certificate
-
ID cards for Trucker Assistance Program

ID card for ScriptSave Drug Discount Plan

Claim reporting guidelines
3.) When is coverage effective for a new contractor?
New contractors shall have coverage as of the date he/she
signs the lease
agreement. No premium is due for that person until the
following month
unless the date contracted falls on the first of month).
4.) Is there pro rata premium due for additions or
terminations?
No.
5.) Can a terminated driver continue coverage under the
Occupational
Accident Program?
No, the Occupational Accident Programs offered by Ameriplan
are not
portable. We view our program as a retention tool utilized by
the motor
carriers in an effort to keep good drivers.
6.) Can drivers of Fleet Owners be covered under the
Occupational Accident
Program?
It depends. Generally, employee drivers of Fleet Owners can
not be covered
under our program. We will not offer Occupational Accident if
the situation
mandates Workers' Compensation coverage be provided. Fleet
Owners who have
drivers who operate and are compensated as independent
contractors may
purchase the Occupational Accident coverage.
7.) Do you accept submissions from other brokers?
Yes, we do accept submissions from retail brokers. However,
not all of our
insurance company partners will accept brokered business.
8.) What is the minimum number of Independent Contractor
Drivers required to implement an Occupational Accident
Program?
We will consider groups with 15 or more Independent Contractor
Drivers.
Start up operations with fewer than 15 drivers may be
acceptable. Please
contact us if the coverage request is for a start up business.
9.) Do you have any prohibited classes of business?
We will not offer a quote to moving van, dump truck and logger
operations.
